• Henry Ford Health's Women-Inspired Neighborhood (WIN) Network: This program pilots an enhanced model of group prenatal care with an added focus on addressing SDOH. Group prenatal care sessions are co-facilitated by CHWs and certified nurse-midwives. CHWs also conduct home visits through babies' first birthdays and continuously guide program participants to relevant resources.
• Kent County's Strong Beginnings Program: Corewell Health serves as the fiduciary for Strong Beginnings, a federal Healthy Start program and collaborative effort of 8 community partners. Among other services, CHWs meet with parents through pregnancy and the first 18 months of their babies' lives and connect families to needed resources at no cost to participants.
• Saint Francis Health System's TeamBirth Program: First piloted at Saint Francis Hospital in 2018, the primary goals of the TeamBirth care model are to provide new approaches to perinatal care and reduce NTSV C-Section rates. TeamBirth operationalizes communication and teamwork best practices through shared planning "huddles" between clinicians and patients.
• Dane County's ConnectRx Wisconsin Program: Launched in 2022 through a multi-sector partnership, the ConnectRx program pilots a fully integrated network of healthcare and social service systems across Dane County, WI. Using a platform developed Epic Systems, the program is designed to identify patients' medical and non-medical needs to target low birth weights and improve birth outcomes for Black women in WI.
