HIA researchers partnered with the Pathways Community HUB Institute (PCHI®) team to explore drivers of success behind their community-based community health worker (CHW) care coordination network that reimburses for CHW services. This case study spotlights ProMedica's collaborative approach in integrating the PCHI® model to address infant and maternal health disparities in Northwest Ohio. The CHW model has impacted health outcomes among African American infants in ProMedica's service region, reducing disparities in preterm birth rates and low birthweight rates.
