Cone Health's Advanced Hypertension Clinic: Through a holistic team-based approach and a comprehensive treatment plan which identifies SDOH risks and underlying comorbidities, Cone Health has been able to provide accessible, comprehensive, and coordinate care for patients with complex needs and individuals from marginalized populations.

Virtua Health and Medtronic LABS' Healthy Neighbor Program: Building off an existing Medtronic LABS care model comprised of a community health workforce and Medtronic LABS' digital platform (SPICE), this strategic partnership hopes to replicate successful outcomes from past pilot sites in reducing disparate outcomes among patients with uncontrolled hypertension and type II diabetes.

IU Health's iHEART Collaborative: In partnership with Novartis Pharmaceuticals Corporation, this initiative convened a collaborative of local community-based, faith-based, academic, and health institutions and develop a program to reduce disparities in cardiovascular health. The program deploys community health workers in neighborhood and clinical settings to provide education on chronic disease management and support IU Health care teams in addressing patients’ unmet social needs.
