Habitat Health, a JV by Kaiser Permanente and the investment firms Town Hall Ventures and New Enterprise Associates, is scaling their PACE-enablement model beginning with two Kaiser markets in 2025.
The company, founded last year, will create JVs to scale Program of All-Inclusive Care for the Elderly (PACE) models, a federal model in which providers receive capitated payments from Medicare and Medicaid for providing a comprehensive medical and social services. The model’s goal is to provide support to nursing home-eligible seniors who wish to age independently at home.
